Hello Kubick and Kubick Family,
After a short closure due to the spread of COVID-19, we are happy to be reopening for modified hearing care services. We will be offering a no-contact drive through for hearing aid cleaning and adjustments. We are also offering telehealth appointments. We will not be offering in-office services for now but will bring that service back as soon as we feel it is safe for both our patients and our providers.
We are holding limited office hours (10am-1pm) on Tuesday to get patients back on the schedule. If you would like to schedule, please contact us at (973) 309-7294 or firstname.lastname@example.org.
NEW OFFICE POLICIES TO LIMIT EXPOSURE FOR BOTH STAFF AND PATIENTS
(please read this section carefully):
We are following guidelines from authorities on proper measures to take for maintaining safe and sanitary conditions at Kubick and Kubick. We are continually wiping down surfaces and treatment spaces as well as practicing good hygiene with frequent 20 second hand washing and using alcohol-based hand sanitizer. We feel confident that we can continue to treat you with the quality of care you have come to expect from Kubick and Kubick amidst this challenging time.
We ask that you arrive on-time for your appointment and wait in your car. When you are here, please check-in for your appointments by calling us at (973) 309-7294. It also helps limit your exposure and supports social distancing recommendations.
We are also implementing a no-touch check in process. If you have a new insurance card, please take a picture of the front and back of the card and send it to the office ahead of your appointment by emailing to email@example.com. If you need to change your method of payment on file please call and provide the new credit card number ahead of your appointment, or the front desk will ask you to read it to them while they manually enter it into the computer.
We also ask that you continue to be mindful and not schedule or keep appointments if you:
- Have any symptoms of illness (especially fever, cough, or shortness of breath) such as sneezing, runny nose, nausea, sore throat or allergies, or have had any of these symptoms in the past two weeks. It is hard to differentiate allergies from more serious illness this time of year. We are trying to keep our patients and staff healthy!
- If you have had contact in the last 14 days with someone who has been infected with COVID-19. Remember even if you feel well, you can still transmit the virus to others.
